• Once you have submitted your online appointment request, please allow 24 hours for a phone or e-mail confirmation of your appointment.

Sending an appointment request is not verification of the date & time requested. A staff member will contact you with confirmation of your appointment. If you submit an appointment request Friday after 5:30pm, Saturday or Sunday, a staff member will get in touch with you on Monday.
